For your peace of mind, here is a helpful checklist to ease the new patient process.

To Send Prior to Appointment:

  • Diagnostic Physician and/or Progress Notes
  • Medical Information and Pathology Slides
  • Diagnostic Films with Radiology Reports
  • Diagnostic Surgical Reports
  • Diagnositc Lab Reports
  • Insurance Information Including Health Plan Cards

To Take to Appointment:

  • Appointment Schedule
  • Copies of all Loma Linda University Medical Center Communication
  • Completed History Form
  • Completed Contact Information Form
  • All Insurance Documentation Including Health Insurance & Prescription Cards
  • List of Your Doctors and Their Contact Information
  • List of Current Medications and Dosages
  • Notebook with Questions to Ask Your Dr. and to Record Notes
  • Advance Directive (Living Will and Medical Power of Attorney) Documents

Personal Items:

  • Comfortable Clothes, Shoes and Jacket
  • Reading Material or Personal Entertainment (iPod w/headphones, etc.)
  • All Medications Needed for Duration of Stay
  • Contact Information for Lodging Location
