Patient Checklist

Prior to Your Appointment:

Before you come in for your appointment please make sure the following documents have been sent to the Center:
•    Diagnostic physician and/or progress notes
•    Medical information and pathology slides
•    Diagnostic films with radiology reports
•    Diagnostic surgical reports
•    Diagnostic lab reports
•    Insurance information including copies of health plan cards

To Bring to Your Appointment:

When you come in for your proton therapy treatment make sure you bring the following:
•    Appointment schedule
•    Copies of all Loma Linda University Medical Center (LLUMC) communication
•    Completed history form
•    Completed contact information form
•    All insurance documentation including health insurance and prescription cards
•    List of doctors and their contact information
•    List of current medications and dosages
•    Notebook with questions to ask your doctor and for taking notes
•    Advance directive (living will and medical power of attorney) documents

Personal Items:

Your treatment period may vary, but in order to make your appointment with us more comfortable plan on bringing the following with you.
•    Comfortable clothes, shoes and jacket
•    Reading materials or personal entertainment (e.g., iPod with headphones, small portable electronic devices, etc.)
•    All medications needed for the duration of your stay
•    Contact Information for lodging location
